+Emergency Care

If you are experiencing a life threatening emergency call 911. If you are experiencing a less urgent problem please call us during our regular business hours of 8am-5pm - Monday through Friday. If our office is closed you will be connected to our 24-hour answering service.

+Prescription Refills

If you are receiving medications from our office, it is necessary for you to comply with medication policy rules provided to you. Please see "forms" tab to review the policy.

+Billing and Financial Policies

It is your responsibility to provide our staff with your insurance coverage and keep us updated with any changes to your coverage. Our office will file all insurance claims on your behalf, including secondary coverages. We will also verify benefits on your behalf.

All co-payments are expected to be paid on the day of your appointment. For your convenience, we accept Visa and Mastercard payment.

If there is a balance remaining after insurance, you will receive a monthly billing statement. Payment is due within 30 days of receiving your statement. If you are unable to pay in full, please call our billing department. We reserve the right to utilize an outside collection agency for unpaid balances.

If you are uninsured, our office does offer special rates. Our billing department will discuss those rates with you prior to your appointment. Self-payment amounts are due at the time of service.

+Work-Related Injuries

If your visit is a result of a worker's compensation or personal injury, it is your responsibility to provide our office accurate information prior to your first visit. This information will include the date of injury, the claim number, the insurance carrier involved along with their address, phone number and contact person. If you have retained an attorney to handle your case, please provide our office with that information as well. We will make every effort to work with the insurance carrier and/or attorney while your case is ongoing.

Dr. Pannu only works with adult patients; pediatric patients are not seen at this time.
